Today, I want to dive into a topic that’s been causing quite a stir in the dental community: the overnight closure of Smile Direct Club (SDC). Let’s unpack what happened, why it matters, and what paths are open for those left in the lurch.

The Abrupt End of Smile Direct Club

Imagine this: You’re midway through your orthodontic journey with SDC, making payments, following the plan, and then suddenly – they’re gone. No more support, no more patient portal, just radio silence. This is exactly what happened when SDC unexpectedly shut down, leaving countless patients stranded without guidance and still on the hook for payments even though they can’t continue their treatment. It’s like being left mid-air on a flight with no pilot.

 

Behind the Fall of SDC

In my view, SDC’s model had a fundamental flaw: a lack of real doctor involvement in guiding patient cases. I saw this firsthand with a patient who, after using SDC, lost an implant. It’s a situation that could have been avoided with proper dental oversight, considering that implants cannot be moved orthodontically. This absence of an initial exam to spot such issues led to a costly $10k surgery and months of healing for her. SDC, in essence, was equipped for minor, aesthetic tweaks, primarily to the front teeth, but not for comprehensive orthodontic care, including bite adjustments or complex cases.

Their marketing strategy I believe also played a role. SDC relied heavily on social media influencers, many of whom already had great smiles, possibly even veneers, to begin with. This approach, along with their narrative that traditional dentistry is unnecessarily expensive, created inflated expectations. However, as many patients realized, SDC was overpromising and underdelivering.

What Now for SDC’s Patients?

For those of you affected by SDC’s closure, the path forward is unclear. With no help from SDC and a lack of access to your dental records, the situation is complex. I foresee lawsuits, but they might offer little solace as SDC dissolves. Sadly, no dentist can seamlessly take over these cases due to the lack of proper oversight and documentation, and your only real option is to start from scratch with your dentist.

Introducing a New Path

But there’s a silver lining. I’m currently developing and testing a virtual Invisalign system at Deering Dental. This system promises the convenience of remote orthodontic care with the detailed and personal attention you expect from an in-office visit with me. It’s a blend of virtual monitoring and necessary in-person check-ups to ensure your treatment is on track. I’m fine-tuning this system with feedback from participating patients to make sure it meets our high standards for personalized care and convenience before launching it early next year.

Final Thoughts

The sudden closure of Smile Direct Club is a cautionary tale about the importance of professional dental supervision. Orthodontic treatment is more than just cosmetic – it’s a health procedure that needs expert oversight. What is the real value of dentistry? Is it just about the price tag, or is there more to it?

Traditional dentistry, primarily dictated by insurance constraints, tends to focus on cheap, short-term solutions. It’s like placing a band-aid on a deeper wound. This approach may seem cost-effective right now, but it can lead to more significant problems and expenses over time. Think of the regular cycle of fillings, root canals, and crowns; something I like to call the “Tooth Cycle of Death”. With each procedure, the dentist is literally chipping away at your teeth until there’s nothing left to save. The traditional approach to dentistry almost guarantees more work will be needed in the future, and with root canals costing anywhere from $600 to $1,500 per tooth and implants anywhere from $4,000 to $8,500 per tooth, the actual cost of traditional dental work over time can be staggering.

Now, let’s talk about biomimetic dentistry. Often perceived as a luxury, Biomimetic dentistry reveals its true value as patients age and discover that earlier treatments they underwent weren’t adequate to preserve their natural teeth over the long term. Take, for example, a guest I had just last week who we’ll call Mary. Mary is one of my more senior guests, so she didn’t have the chance to take advantage of the tooth-preserving benefits of biomimetic dentistry early enough in life when it could have made a difference. After decades of traditional drill and fill dentistry, Mary didn’t have enough natural tooth structure left to save, and so her best option was extensive implant work. Still, she could have bought a new BMW or taken an extravagant European vacation with the money she’s spending.

The car is loaded.  You’re 75% sure you have everything you need. The kids are already strapped in their seats as you pace through the house with a lingering “Did I forget something?” feeling. You haven’t left the driveway yet and you’re already an hour behind schedule. You look at your husband and with just your eyes you both say to each other, “Why did we want to do this again?” as dreams of serene campfires are overshadowed by the imminent chaos in the car. 

We’ve been there and as we are currently planning our next road trip with our 4 girls ages 7, 5, 3 & 1, we thought we’d share a few tips we’ve picked up along the way.

  1. Embrace the Boy Scout Motto (Always be prepared)

Two weeks before any road trip, we sit down for our planning ritual. Peter and I list everything (and yes, we mean everything) and split tasks. This prevents last minute snafus like the time Peter forgot the trailer hitch keys? Peter’s quick metalworking fix saved the day, but who needs that stress?

  1. Comfort is Absolutely Key

Those long drives can be tough on little ones. We’re big on making sure our kids are snug with pillows, blankets, and strategically placed luggage for footrests. It’s all about avoiding those uncomfortable sleeps and ensuring they’re cozy enough to doze off for as long as possible.

  1. Keep the Little Ones Busy

A bored child is a recipe for disaster. Each of our kids gets a personalized bag of goodies – tablets loaded with movies and audiobooks, coloring books and colored pencils (Crayons melt in hot cars), a new toy, and mess-free snacks. Don’t forget the classic car games! ‘I Spy’ and ‘License Plate Bingo’ are Pinto family road trip staples.

  1. Don’t Forget About Mom and Dad

This is your vacation too! Pack things that make you happy and comfortable. I never leave without my sleeping mask, neck pillow, and headphones. When I need a break, I just tune out and let Peter take the lead.

  1. Accept the Inevitable

We’ve learned to accept that things will go off-plan. Kids need breaks, toddlers have meltdowns, and yes, something will probably be left behind. But remember, there’s always a store along the way to pick up forgotten items and if you get stuck in traffic, just picture the wonderful memories waiting to be made at your destination.

Here’s to making unforgettable family memories, one mile at a time.

The Domino Effect: Declining Trust, Declining Health and How to Fix It

Have you ever watched an old movie or TV show where the local doctor made a house call? My daughters and I love watching old episodes of Little House On The Prairie, and of course, one of my favorite characters is Dr. Baker. He knows every family in town on a deeply personal level, and the trust everyone has in him is obvious. He walks into their homes, and you can tell he’s not just a doctor, but a trusted family friend.

That feeling of a trusted family friend was one of the reasons Peter and I started Deering Dental. The modern healthcare industry has become too focused on test results, profits, and insurance stipulations. Add those factors to the ever-increasing costs to provide quality care, and the picture makes a lot of sense. Doctors are being forced to spend a lot more time doing things other than caring for patients, overhead is going up, and getting reimbursed by insurance has become a never-ending battle. All this has led to what I call fast-food medicine. Serve as many people as possible, as cheaply as possible, and hope that at the end of the day, you’ve made enough to make payroll.


With most doctors being forced into this high-volume way of practicing medicine, it’s no wonder surveys have been showing a decline in trust. In the past, doctors may not have had all the fancy gizmos we have nowadays, but they also didn’t practice in a manner that’s more akin to a factory worker. That ability to spend time with you makes the difference because to genuinely care for someone, you have to know them. Test results alone can’t give you the whole picture of what’s going on, which means if your doctor can’t take the time to speak with you, they won’t be able to provide the best diagnosis and treatment options for you.


Trust in doctors is on the decline, and mounting evidence supports my hypothesis that this decline in trust is leading to declines in health.

For example, in Trust in the Health Care Professional and Health Outcome: A Meta-analysis, the authors concluded, “Patients reported more beneficial health behaviors, less symptoms and higher quality of life and to be more satisfied with treatment when they had higher trust in their health care professional.”


I’ll give you an example of what a lack of trust leads to. Recently, I got to sit down with my new guest, Andrea. As we spoke and I had the chance to listen to her story, it appears her last doctor wanted to focus on doing implants and didn’t seem interested in addressing her other concerns. His focus was on what he wanted, not Andrea’s concerns. Andrea wasn’t sold, but those seeds of distrust kept her away from the dentist for some time. After a thorough exam, I determined there are definitely urgent problems that need to be addressed; as for the implants, they may be required in the future, but they’re not necessary right now and will have no immediate effect on the health of the rest of her mouth.


My friend Dennis is another excellent example of how important trust is. After a string of bad experiences, Dennis had had enough and avoided the dentist for years. Things finally reached a point where Dennis knew he had to do something, and luckily he found us. Dennis and I had a long discussion where I got to understand his history, hygiene habits, diet, and anything else that might help in crafting an ideal treatment plan. I gave Dennis two options, and to his surprise, the ideal option would save him 5,000 dollars. When he asked me in disbelief how the ideal option was cheaper, I explained to him that by straightening his teeth with Invisalign first, I would be able to address his issues with less drilling and fewer crowns. The process would take longer, but the results would be more ideal and longer lasting. When Dennis understood that I valued his health above all else, we built enough trust that he was ready to move forward and fix the damage from years of avoiding the dentist.


So what exactly is trust, and how can doctors build more trust with their patients? According to researchers Roman Lewandowski & Anatoliy Goncharuk, Restoring patient trust in healthcare: medical information impact case study in Poland, “Interpersonal trust is characterized by intimacy and closeness, and relates directly with people we know personally, whom we recognize by name, and with whom we interact in a face-to-face manner.”


When was the last time you went to the doctor, and could describe your experience in that way?

I hope when you think about your visits with me, that’s how you feel, and if it is and you have friends that would appreciate the same level of care and attention, send them our way. I promise to take care of them in the same manner I’ve cared for you. I know we’re just one little dental practice. Still, I really believe that if we treat you and everyone else who walks through our doors with kindness and genuine caring, we may be able to change what people expect from their doctors and, that way, push more physicians to change course and focus on what really matters, You.

Working Together

Working Side by Side: Balancing Love and Business

a Confident Smile

The other night, Yenile and I were just lying in bed, chatting about our day – something we do a lot. And guess what came up? The question we get at least once a week: “How do you handle being married and running a business together?” This question is usually followed up with a comment along the lines of, “If I worked with my spouse all day, we’d probably drive each other crazy!”

We got to talking about the last nine years – the good, the bad, and everything in between. It got me thinking, maybe our guests would like to hear our story? Now, Yenile’s a bit more private when it comes to our life outside of work, but after some back and forth, she agreed that it might be fun to let our Deering Dental family see what goes on behind the scenes. So I wrote down a few questions and we conducted an interview of ourselves to share with you.

So here it is, a real talk based on that chat we had in bed. We hope by sharing our story, you’ll see how working together has actually made our bond even stronger – and get a sense of the passion we bring to our practice every day.

What made you guys think of starting Deering Dental together?

Dr. Pinto: Honestly, I was just tired. The places I worked at didn’t match what I wanted for my patients. The way people were treated, both the patients and the team, it was just about maximizing insurance payouts and profits instead of doing what was best for the patient. But it was Peter, seeing me come home upset day after day, who suggested we do something about it.

Peter: Yeah, I saw how much she cared and thought, “Why not make our own place? You do the dental stuff, I’ll handle the business side.”

Dr. Pinto: And just like that, with Peter backing me, Deering Dental started becoming more than just a dream. We wanted a place where the patient truly came first.

Peter: We put in a lot of hours researching, interviewing everyone we could about what they didn’t like about going to the dentist and then working on solutions to each issue. Yenile was clear she didn’t want to cut corners on care, and I was on board with that. My job was to figure out how to make it work as a business.


You two work and live together. How do you not drive each other crazy?

Dr. Pinto: We have our moments! But really, we balance each other out. And even though we’re in the same building, Peter’s office, his “dungeon”, gives us some space.

Peter: From the start, we decided on our roles. Yenile is the dental expert, and I focus on the business. We trust each other to handle our parts, and I think that’s why it works.

Dr. Pinto: It’s true. If you work with your spouse I recommend having boundaries. We talk, we listen, and most importantly, we respect each other’s space and decisions.

Peter: And, honestly, being married helps. We’ve learned how to work through disagreements in our personal life, and it carries over to the business.

 


But you must have disagreements. How do you handle those?

Dr. Pinto: We do, like any team. When we disagree, we talk it out, weigh the pros and cons, and usually find a middle ground. Sometimes, one of us feels really strongly about something and we let that person take the lead. We call it the, “It’s important to me card”. There’s no formal limit to it, but we’re careful not to play that card often.

Peter: When used sparingly, the, “It’s important to me card” is a great tool you can use. Like when Yenile wanted to go all-in on Biomimetic dentistry. I won’t lie, the numbers were tough initially, but she believed it was best for our guests and after diving into the scientific research I had to agree with her.

Dr. Pinto: It was so so much better than everything I had been taught in school, that I just couldn’t back down. It was too important. So I used my card and we had to figure out how to make it work.

 Peter: Right. So I took another look, tweaked a few things, and found a way to support her vision. Communication’s key. We might not always agree instantly, but we trust each other to come to the best decision.

Dr. Pinto: And that trust? It’s at the heart of everything we do at Deering Dental.


What were some bumps in the road when you started Deering Dental, and how’d you get past them?

Dr. Pinto: Well, we’ve seen a lot, haven’t we? From dealing with the aftermath of hurricane Irma that literally destroyed the office, to all the craziness with COVID. And don’t get me started on the insurance stuff. But we’re both problem-solvers, so we always found a way through.

Peter: You’re right. My time as a firefighter taught me a lot about handling emergencies. But every issue was different. Like after the hurricane, we were working out of whatever space we could find. We always just talked it out, did our homework, and made a plan. There were a few times when we weren’t sure things would work out, but we never gave up, plus you’ve always trusted me during those SHTF moments to McGyver our way out of it.


What might surprise people about what goes on behind the scenes at Deering Dental?

Peter: Some folks might think I’m just in the background, but I’ve been in the thick of things.

Dr. Pinto: Totally. People might not know that Peter used to be right at the front desk. He’s been our rock. Even if he’s not as visible now, he’s still everywhere in the practice.

Peter: Thanks, Yenile. I always just tried to be there, supporting her ideas and making sure we show our real selves, whether it’s on social media or in our ads.

Dr. Pinto: And let’s not forget your awesome ideas. Like how you made us all start referring to everyone as our guests instead of patients, the amenities menu or the special way we greet everyone. Peter has some hospitality experience and thought we should treat our guests like they’re at a fancy restaurant, and it’s been a hit. When your spouse has life experiences you don’t, it can be a real asset. Peter’s eye for service and attention to detail has been a big help.


How has working together affected your personal lives?

Dr. Pinto: We learned early on that we needed to switch off from work at home.

Peter: Absolutely. When you work with your spouse, business manages to sneak it’s way into everything, so you have to have boundaries before it consumes your entire relationship. I remember setting the “10 O’clock rule” – no more business talk after 10pm. It was super important before we had our kids and were always buzzing with energy. The work talk never stopped and Yenile was starting to feel too much like my business partner and not enough like my wife.

Dr. Pinto: I’m so glad we did that. And as our family has grown, we’ve figured out how to juggle everything. As the dentists I don’t have as much flexibility, so Peter taking a more behind the scenes role has allowed him to take care of a lot at home.

Peter: I’ve gotten into the groove of balancing work and handling things at home, school pick-up and drop-off, a million other things and just making sure everything runs smoothly.

Dr. Pinto: It’s like a dance we’ve been doing together. Our dental practice feels like part of our family. We always want to make our patients feel like they’re family too.

Peter: Working together, going through the good and the bad, has made us stronger as a couple. It’s all about keeping our connection strong, talking things out, and being there for each other.


Got any tips for other couples thinking about working together?

Peter: We took this test, the Kolbe test. It really helped us see where each of us shines. Once you know how you both work best, you can divvy up tasks better. My wife would agree it was a game changer for us.

Dr. Pinto: Communication is everything. Make sure you’re both on the same page about what you want. And keep talking, about work stuff and personal stuff. We’ve learned that most of our disagreements over the years have stemmed from poor communication and what Peter likes to call, “Unspoken expectations”.

Peter: Remember to keep the romance alive too. We set rules like the “10 O’clock” one, and they help. At the end of the day, the love you share will reflect in your business. If you work with your spouse your personal relationship is the core that everything else is built on, so you have to take that relationship seriously and always work on it, because if things are not right at home your business will suffer as well.

Dr. Pinto: Absolutely, and don’t get so wrapped up in work that you forget why you started this journey together. Regular dates, quality time, and just respecting each other have made all the difference for us. It’s all about protecting that bond.

As a mom of four girls, the Tooth Fairy is almost a regular visitor at our home. My husband Peter had an ingenious idea to make this magical being more than just a money-exchanger for lost teeth. When our oldest, Francesca, began losing teeth and struggling with reading at the same time, Peter had an idea, “Why not enlist the Tooth Fairy’s help?”

The timing was perfect. Homework was turning into a daily battle of wits between my husband and Francesca and he didn’t feel like he was gaining ground, so after Francesca lost her first tooth, Peter came to me with a note he had cut into the shape of a tooth that read: “Francesca, congrats on losing your first tooth, I’m really excited to be your tooth fairy. I heard you’ve been having some trouble with reading and wanted to let you know that I had the same problem when I was in fairy school. Luckily I kept trying and reading became fun and easy, so don’t give up. I know your birthday is coming up in a week, keep an eye out, because I have a special gift coming for you. Your friend, T.F.”

The note along with a few dollars had an immediate impact. Francesca tackled her homework with gusto and even received her Tooth Fairy’s favorite book for her birthday, which she devoured eagerly. Now, the Tooth Fairy is a pen pal offering life advice, and our younger girls are eager to start losing teeth and meet their own Tooth Fairy pen pal.

Jorge knew he had some major dental issues to fix. His smile was getting crooked and yellow but for years he just put it off and put it off. Eventually he got fed up feeling embarrassed in front of his clients. So, one day he buckled down and said, “I’m finally ready to do something about this!”

But no one seemed to want to tell him the whole story. He visited several dentists and they told him, “You have some gum issues we need to sort” or “You need a few teeth pulled.”

Jorge knew his issues were more extensive than that. “But no one would give me the whole picture of what it was going to take to get healthy, restore my smile and get my confidence back.”

Jorge’s complaint is one of the most common I hear about dentists. It goes something like this… “Every time I go to the dentist they find something new that has to be fixed. I feel like it never ends. I feel like I’ll always be going to the dentist for a procedure…not just cleanings and checkups! Are my teeth really this bad? Or is my dentist just incompetent and keeps missing things?”

Most people realize that dental problems don’t happen overnight. “If I have a big cavity now, shouldn’t my dentist have spotted a small cavity when I was here six months ago?”

Truth is that your dentist most certainly DID spot that small cavity. But decided to wait to tell you.

(Now, let me just say that only unethical dentists will have you fix something that’s not “broken.” There have been scandals involving dentists doing unnecessary procedures, but I’m not talking about them. I’m talking about ethical, competent dentists who “spread out” the dental work you need.)

How does this work? Well, if a guest comes into the office and we discover they need a lot of work, SOME dentists are afraid of telling you the WHOLE truth. They think that if they tell you everything that’s wrong, you’ll jump out of the chair, run out the front door and never look back. Or you’ll simply not believe them. Since no other dentist has ever told you the whole truth, they assume you’ll think, “This dentist is making all this up just so they can do more work and charge me more money!”

So instead the dentist just tells you what he thinks you can handle or afford at the moment.

He doesn’t tell you the whole truth…just part of the truth. It’s not unethical…but it is misleading and annoying and potentially dangerous.

Because with almost all dental problems, the sooner you have them fixed, the easier and less expensive they are to fix. Every week or month or year you lose means the problem gets worse and worse and worse. It’s like recommending a new coat of paint for you house and meanwhile your foundation is completely shot and your house is slowly falling apart.

After a while, your relationship with your dentist gets eroded by distrust and skepticism. It’s a lose-lose-lose situation.

And this “spread it out” approach is more common in my industry than I’d like to admit. It’s so common in fact that it is a frequent topic at dental seminars all over the country. Dentists everywhere are scared to tell you what’s wrong and what you can do about it. So, they make your decisions for you.

For example, my husband Peter started getting cavities caused by normal teeth shifting.The shifting made it nearly impossible for Peter to floss properly. So food would get stuck in his teeth.

Why Human Interaction Still Matters in an AI-Dominated World

AI is often touted as a panacea set to revolutionize everything, promising a dramatic improvement in our lives, but does it really deliver on that promise?

This week, I found myself scanning and bagging my own groceries at Publix and doing the same at The Home Depot. I also needed to handle a few things at the bank where the single client representative they had kept suggesting I handle my banking myself online. I expended an absurd amount of time attempting to resolve an issue with Amazon, a two-minute task had a real service representative been available.

Google informed me that their AI mistakenly flagged our business account as a duplicate, subsequently erasing every review response I’d written over the past eight years, and with no means to restore them. To top it all off, a lunch with my husband at a formerly full-service restaurant required me to effectively be my own server via my phone. However, this didn’t prevent them from suggesting a 25% tip for self-service. Call me skeptical, but I’m struggling to see this as progress.

I understand that times are tough. Inflation is soaring, non-essentials are becoming a luxury, good employees are hard to find, and businesses are trying to accomplish more with less. However, it’s increasingly difficult to justify paying more while receiving less, especially in the service sector, which appears determined to discover new ways of serving without providing any actual service.

But here’s the thing: human beings are inherently social creatures. We crave interaction. It’s an essential part of our nature, and we cannot thrive without it. The impacts of solitary confinement and the mental health surge after three years of Covid-19 are ample evidence that we require real human interaction. For those people and businesses that understand this fundamental aspect, AI could be our greatest ally.

How can something seemingly eroding human interaction be beneficial? As I explained to my team last week, AI can’t offer you a comforting smile. AI can’t see that you’re nervous about your dental appointment and take steps to put you at ease and make you comfortable. AI can’t celebrate and share in the emotional excitement of seeing your newly restored smile for the first time. Only humans can provide that level of empathy and connection. As AI permeates more aspects of our daily lives, the value of human interaction will only increase, and we’ll seek it wherever possible.
